Referrals

If you would like to have a sleep test, please speak to your family doctor about initiating a referral. The referral form can be completed and signed by your family doctor. Once your referral form is received by our office, you will be contacted to schedule an appointment. Your family doctor will determine if a consultation or sleep study (or both) is required.
